What is it?

Support Coordination (SC), is a Capacity Building support. The SCs role is to:

  • assist you to understand your plan
  • help you take action toward your NDIS Goals
  • connect you with support providers including allied health, home & living, assistive technology suppliers, nurses etc 
  • support you by coordinating your support team so that it meets your needs
  • monitor your progress and gather evidence 
  • liaise with providers and the NDIA by updating them on any changes in your life, sending progress reports, attending stakeholder meetings, reassessments and reviews
  • advocate for changes to your supports where evidence supports
  • manage safeguarding risks and complex system overlays
  • assist you to manage your support budget so you get the most from your plan

Does everyone have a Support Coordinator?

No. Support Coordination (SC) is a stated item found in the Capacity Building category of your plan. It must be in your plan to be eligible to use a SC. If you don't have a SC, you can talk with your appointed Local Area Coordinator(s) LAC (if you have one), or directly with the NDIA. 

SUPPORT CO LEVEL 1

Does everyone have a Support Coordinator?

If you have SC Level 1 you are considered to be reasonably high functioning and require a basic level of support to navigate the early stages of your Plan implementation. 


LEVEL 1 DUTIES

  • Develop a sustainable budget that lasts the length of your Plan  
  • Make referrals for your supports
  • Connect you to providers
  • Help you understand  & sign service agreements 
  • Inform you of your rights & obligations as a NDIS consumer
  • If funding permits, check in for progress updates 
  • Submit evidence,  reports and assessment outcomes to NDIA for review

SUPPORT CO LEVEL 2

If you require a SC Level 2, NDIA has assessed that you have moderate functional capacity and require assistance to connect, coordinate & monitor your supports. 


LEVEL 2 DUTIES


  • Everything in LEVEL 1 plus
  • Develop Support Plan
  • Coordinate support schedules, appointments, assessment bookings, 
  • Tracki shift notes, logs, risk register, incudent reports and progress to evidence outcomes 
  • Periodic check ins and reporting obligations 
  • Prepare documentation for reviews and reassessments 
  • Coordinator stakeholder meetings 
  • Support access to mainstream & informal support

SUPPORT CO LEVEL 3 (SPECIALISED)

PSYCHOSOCIAL RECOVERY COACHING (PRC)

 If you are seeking SC Level 3 you are considered high, complex needs, with low functional capacity,  who will likely need significant support to navigate the NDIS. 


LEVEL 3 DUTIES

  • Everything in LEVEL 1 & 2 plus
  • Assistance navigating multi-layered complex system overlays with other government agencies such as Justice, Public Guarduan, Public Trust, Public Housing, Health, Child Safety etc
  • Support with extensive Risk Assessments, risk profiling, safeguarding risks and escalations via the appropriate channels 
  • Engaging high complex supports to manage PEG feeding, mealtime management, behaviours of concerns etc
  • Evidencing system and administrative failures that adversely impact you
  • Assistance and support through crises within the parameters of the SC Role
  • Advocacy in choice & control, system navigation & participant rights under the NDIS framework

PSYCHOSOCIAL RECOVERY COACHING (PRC)

PRC is practical, recovery-focused support that helps people with mental health conditions strengthen independence, social connection, and wellbeing


PRC DUTIES


  • Recovery-Focused Coaching (Not System Navigation)
  • Daily Living & Functional Capacity Support
  • Build routines, coping strategies, motivation, and emotional regulation linked to mental health impacts on daily life
  • Develop, review, and implement recovery plans that align with psychosocial disability impacts
  • Coach participants to self-manage appointments, medications (non-clinical) wellness strategies, and social participation
  • Provide ongoing, relationship-based support with regular engagement to build trust, insight, and confidence
  • Understand and integrate clinical advice into everyday life, without coordinating or managing services in the way SCs do


Our dedicated PRC is experienced in Mental Health recovery & has lived experience of mental health impacts. She understands first hand the struggle of navigating life during times of distress. .
